RegionalOne Credit Union offers two ways to apply for a MasterCard.

An application form can be completed when you call into your local branch, or you may prefer to print the application form below and bring the completed form with you.

Download and print out an application form. Once you have completed this form you can mail, fax or bring it into one of our branches, or mail it direct to Citibank at the address shown on the application.
