Print out and complete the necessary form (see below).

Return it to us:

• bring the form into your nearest branch
• mail the form to us at: PO Box 1000 Bendigo VIC 3550

We'll get back to you with your new cheque book once it's been set up and the cheque forms printed- it's that easy!
